Hi, my name is Man Hong Wan. I am currently enrolled in the Nursing Informatics Program at CUNY School of Professional Studies. I started my nursing career in 2014, working for NYC H+H/Gouverneur on a sub-acute rehabilitation unit as a staff nurse. The patients in my unit were primarily post-surgical patients who have undergone total knee and or total hip replacements. My team members and I were responsible for providing the residents with the utmost pleasant healthcare experience during their rehabilitation. In April 2018, I transferred to Community Care (formerly At/Home), a home health division of NYC H+H. I performed assessments at the patients' homes post-discharged from hospitals or rehabilitation centers. I manage their cases by implementing evidence-based care to ensure their recovery was optimal. My goal is to empower all my patients through prevention and teachings of medications, infection-control methods, fall prevention, emergency preparedness, and self-manage their diseases.
